Part-time Service Coordinator Hialeah

Job Summary

The Service Coordinator will serve as an advocate for seniors living in HUD 202 complexes.

 Credentials, Experience, and Abilities

  • Bachelor’s Degree in human services or related field plus 1 year experience working with the senior populations, or High School Diploma and 4 years related experience.

  • Ability to communicate effectively and work independently.

  • Administrative self-sufficiency including computer proficiency.

  • Possess valid driver’s license and safe driving record.Must meet insurability standards in order to drive.

  Essential Functions

  • Perform assessment of resident’s service needs and update as needed, at least annually.

  • Monitor resident’s status and provide service information and referrals as needed or requested. Train residents in self-advocacy skills. Arrange for guest speakers and health clinics based upon resident needs.

  • Document service coordination activities.Maintain confidential resident electronic, paper and up-to-date statistical records in compliance with agency standards and funding requirements.Participate in peer reviews.Prepare monthly and quarterly reports as required.

  • Maintain positive relationships with community agencies, residents, their families, staff and all other related persons.

     

Normal Working Hours and Conditions:

  • 20 hour or more workweek, with meal and rest breaks as mandated by law.

  • Weekends or evenings as required.

  • Some travel may be required.

Rate of Pay:
This is an hourly position at $15.57 per hour.
